Danaparoid sodium (trade name Orgaran ®, manufactured by Essex Pharma) is a drug from the group of heparinoids, the blood clotting - similar to heparin - inhibits. It consists of a mixture of glycosaminoglycans, which are obtained from Schweinedarmmukosa. The average molar mass of about 6 kDa.
Composition
The drug consists mainly of heparan sulfate ( Suleparoid sodium, about 84 %). Further contains danaparoid sodium dermatan sulfate (sodium salt of chondroitin sulfate B, about 12 %) and a small proportion of chondroitin sulfate ( chondroitin sulfate as free acid, about 4 %).
Dermatan
Chondroitin sulfate
Only 4% of Heparansulfatmoleküle have a high binding affinity for antithrombin III. Danaparoid although similar heparin is recovered from Schweinedarmmukosa, the mixture contains no fraction of heparin.
Application
It is given for the prevention of deep vein thrombosis in situations where heparin should not be used. These are usually patients where lack of platelets by heparin is caused the so-called heparin - induced thrombocytopenia II type ( HIT ).
Contraindications
Not or only after consideration it may be used in people who already suffer from a bleeding disorder, uncontrolled high blood pressure, severe renal or hepatic insufficiency, gastric or duodenal ulcers or hemorrhage. Should serious bleeding occur, the effect can be mitigated by transfusion of fresh frozen plasma. Regular monitoring of platelet count is necessary.
