Polidocanol (trade name Thesit ® ) is a surface-active substance with a oberflächenbetäubenden effect. Polidocanol is used as a drug, as a pharmaceutical excipient and as a cosmetic Inhaltststoff.

Medicinally polidocanol is externally effective in itching, or mild pain, also it is used for sclerotherapy of varicose veins ( varices ), spider veins and hemorrhoids, to which it is injected into the appropriate tissue.

Physico-chemical properties

Polidocanol is a polyalkylene glycol ethers. Polidocanol used medicinally ( lauromacrogol 400) is by definition of the European Pharmacopoeia 6.0, " a mixture of ethers of various Macrogols with fatty alcohols, mainly lauryl alcohol. " Mean fatty alkyl chain length is 10 to 14 carbon atoms; the average number of oxyethylene units is from 7 to 11, corresponding to a relative molecular weight of 400 for the polyoxyethylene portion.

Due to the lipophilic and the hydrophilic dodecyl ether chain results in a good miscibility with water, oil and other solvents. In addition, polidocanol has surface-active properties as a good wetting and thus a rapid cleaning power.

Therapeutic Areas of application

In three to five percent concentration polidocanol is used for pain and itch relief in dermatological formulations ( ointments, creams or gels ), such as eczema, itchy skin and eczema. It may also be combined with other drugs. Also in finished drug products and medical skin care products ( concentrations up to 3 percent) comes polidocanol used.

For sclerotherapy for spider veins or hemorrhoids polidocanol used in 0.25 to 3 % concentration in ethanolic- aqueous solution that is injected intravenously or submucous. Polidocanol is up to 4% used in rinse-off products such as shampoos and washing lotions in concentrations as nonionic emulsifier and wetting agents. Because of the short skin contact up to wash a local anesthetic effect is excluded.
