The headwaters of the river lies in the Kuskokwim Mountains. It flows northeastward 37 km south-west from Manley Hot Springs to its confluence with the Tanana River. The Cosna River is part of the river system of the Yukon River.
The name goes back to the 1899 by Lieutenant JS Herron documented name of the Tanana, an Alaska Native people, for the river.
