Dermatome (instrument)

A dermatome ( v. AltGr δἐρμα. Derma Skin and τομή tomé section) is a surgical cutting instrument for obtaining uniform thickness skin flap transplantation for free. The obtained skin areas find their application in trauma, burns or burns. In dentistry the mucotome is used to ablate mucosa grafts.

Types of dermatomes

Dermatomes can be operated manually or electrically. In the 1930s, the first manually operated Trommeldermatome were developed. Air powered or electric dermatomes are more precise when explanting long strip of skin with homogeneous thickness. With the dermatome to skin surfaces can replace up to 80 square centimeters in a train. The Elektrodermatom with which you can peel off the skin in long strips, is used in the treatment of burn wounds. Manual dermatomes (eg knife after Thiersch, Padgett Trommeldermatom, Blair Humby dermatome ) are not well suited for larger donor sites. When using the mesh or net flap technique ( mesh graft transplants ), a split-thickness skin flap is provided by the cutting of a roll - mesh graft with a dermatome pattern.

Mucotome in dentistry

Analogous to the dermatome is used in dentistry, the mucotome to ablate mucosa (mucous membrane ), which serves as a Gingival graft. This is to broaden the keratinized ( attached ) gingiva prior to the inclusion of dentures ( Präprothetik ), applied to the supply of dental implants and to Receding gums ( gingival recession ) diseased teeth. A distinction is made between the Handmukotom after Deppeler, with the disposable blades of different widths ( 7-16 mm) is fitted and the motor-driven mucotome after Mörmann, can be obtained with the mucosal flap of 6.5 mm width and 0.75 mm thickness. Following the grafts can be thinned with the mucotome cut under Ringer solution with a scalpel in the required form or on a sterile glass if necessary.
