Prokonvertin (English: Proconvertin or serum prothrombin conversion accelerator ( SPCA ); Synonyms: stable factor, and clotting factor VII Prothrombinogen ) is an enzyme involved in blood clotting. The synthesis takes place in the liver and is dependent on vitamin K. Proconvertin has a molecular mass of 59 kDa and belongs to the class of β - globulins.

Genetics

The gene is located on chromosome 13 for Proconvertin ( 13q34 ).

Physiology

The main function of Proconvertin the activation of the coagulation process. If there is a breach of the blood vessels, so the existing tissue tissue factor ( factor III, Gewebsthrombokinase, tissue thromboplastin ) enters the blood and activated there, along with calcium, Proconvertin. This activated Proconvertin (factor VIIa) now activated, together with calcium and phospholipids, factor X ( Stuart - Prower factor) and factor IX ( Christmas factor).

Diseases

Diseases whose cause is a mutation of the Proconvertins are rare (1 in 500,000 in the general population ) and are inherited in an autosomal recessive manner.

Therapeutic use

Recombinant Proconvertin ( NovoSeven ®) was introduced as a therapeutic measure in uncontrollable bleeding in hemophilia patients, the inhibitor had formed against replacement coagulation factors.

It may be used in uncontrolled bleeding. The hope is that it is only there to initiate clotting, where thromboplastin already exists. However, studies have shown an increased risk of deep vein thrombosis, pulmonary embolism and myocardial infarction with the use of recombinant factor VIIa. Another disadvantage of this therapy is its extremely high price.
