Nasal septal hematoma

In a septal hematoma, Nasenseptumhämatom or nasal septum hematoma is a bleeding between perichondrium and cartilage or between the periosteum and bone. This leads to a swelling of the nasal septum. Nasal breathing may stop completely by the bulging of the septal mucosa. The septal hematoma is a relatively common complication of broken nose ( nasal bone fracture ) and a rather rare complication after septoplasty. In children Nasenseptumhämatome are more common than in adults.

Since the perichondrium that supplies the cartilage by diffusion, is lifted by the hematoma, the cartilage may no longer be adequately fed. This may on the one hand to form a Septumnekrose and consequently the formation of a so-called saddle nose come, on the other hand the risk of infection is increased. An infection of Septumhämatoms leads to Septumabszess, the hematogenous across the cribriform plate to the cavernous sinus and spread a dangerous sinus thrombosis as well as meningitis ( meningitis ) can cause.

The septal hematoma is diagnosed by mirroring the nasal cavity ( rhinoscopy ). The treatment consists in Septumhämatomen pronounced in the surgical removal of the hematoma. With the aid of a scalpel to cut the mucous membrane of the nasal septum ( incision ) and the hematoma is removed by suction. About the placement of a silicone film strip is the drainage. A fixed nasal packing both nostrils prevents a re- emergence of the Septumhämatoms by compression. In Septumabszessen antibiotic treatment is indicated.
