Prifinium bromide is a drug selected from the group of 3- pyrrolidone derivative, which is a synthetic parasympatholytics. It is used to treat spasms of the esophagus, gastrointestinal tract and urinary tract. Currently registered in Germany only preparations for veterinary medicine.

The pharmacological effect is that of the Butylscopolamins. It competitively inhibits the action of acetylcholine at the muscarinic receptors, wherein a relatively strong binding to the M4 receptor subtype. This leads to an inhibition of the parasympathetic influence on the smooth muscle, cardiac muscle, glands and ganglia of the peripheral nervous system. A quaternary ammonium compound it enters only in a very small extent in the central nervous system.

Contraindications for use are glaucoma, enlarged prostate, bowel obstruction, pyloric stenosis and other narrowings in the gastro- intestinal tract as well as cardiac arrhythmias with increased heart rate. In liver or kidney disease, the agent should be used only under strict indications.

As a side effect, a dry mouth and a pupil dilation may occur.
