The disaster machine in 2009

On April 2, 2012 accident flight UT 120 of UTair shortly after launch in Tyumen, with 29 passengers and 4 crew members were killed. 10 passengers survived the crash seriously injured. The plane was on its way to Surgut.

Aircraft

In the plane it was an ATR 72-200 with the registration VP - BYZ. The age of the machine was 9 years and 5 months.

The accident

The aircraft reached an altitude of 210 meters, turned 35 ° to the left into a bank and then by 50 ° to the right. The aircraft crashed at 7:44 local time clock, proposing approximately 2.6 miles beyond the runway and went up in flames.

Causes

The flight data recorder was found in good condition. According to official statements by the investigators both engines were still working normally until impact.

The aircraft had previously been parked 8 hours outdoors to snowfall and temperatures below freezing. In the 90 minutes before the accident 9 aircraft took off without any problems, all of which had been de-iced. In one of them - also an ATR -72 - 3 cm of ice and snow were found prior to its de-icing on the wings and tailplane.

The misfortune machine has not been de-iced prior to launch; the crew did not insist on a de-icing, although snow and ice accumulations saw when rolling. These deposits are still visible on photos after the crash. The aerodynamic properties of the machine were thus so deteriorated that it was the climb to the stall, the pilots did not recognize as such.

Contributing Factors: - Serious deficiencies in the safety management of UTAir, based on documentation and training of crews and ground personnel. - The ground crew of the UTAir in Tyumen was for clearance neither licensed nor qualified. It had only received a 30-minute briefing. - The pre-flight inspection by the ground staff was conducted without the use of ladders, so that the deposits could not be seen. The machine has been reported as "clean". - The pre-flight inspection by the Captain was completed only very fleetingly in a few seconds. - The English language skills of both pilots were so bad that they were not able to understand the contents of the operating manuals and training materials. - Possible fatigue of the crew by violating the provisions on flight duty and rest times.
