Bone cyst

A bone cyst is a benign tumor-like bone lesion which liquid-filled and sometimes a represents cell- filled cavity in the bone. Radiologically, it can be seen here in the radiograph a one-or multilocular radiolucency. The cysts in juvenile bones are usually discovered as an incidental finding during X-ray after an accident or injury as a cause of so-called pathological fractures ( fractures in reduced bone strength).

We distinguish:

  • Simple (juvenile ) bone cyst: Benign, primary unilocular, fluid-filled cavity, usually in the metaphyseal medullary cavity. Will usually conspicuous only by spontaneous fracture. Classic radiographic signs is falling fragment -Sign.
  • Aneurysmal bone cyst ( HID): Benign multilocular bone lesion which either de novo as a primary or secondary access code as concomitant findings of other bone processes (even malicious! ) Arises. Classic findings in MRI is the "Mirror formation " of the cyst contents, ie fluid - fluid levels with different signal intensities.

Therapy

The therapy is critically dependent on the fact of observation: If a bone cyst found as an incidental finding during x-ray, should possibly be made after further clarification by magnetic resonance imaging, a biopsy sample to have been backed Zystenart (juvenile or aneurysmal KZ) followed by adequate treatment.

  • Simple (juvenile ) bone cyst: The established method today is the intramedullary osteosynthesis ( Prevot nailing ) of the bone. Here, a resulting, in some cases fracture is stabilized, the bone cyst same time receive a stimulus to heal. Some groups use to heal not fractured juvenile cysts nor an injection of cortisone solutions or perforation with the opening of the cyst wall.
  • Aneurysmal bone cyst: This is performed surgical removal of the cyst after confirmation of the diagnosis from the biopsy and conducted a replenishment of the cavity with bone cement. The course is controlled. If after 2 years of no new tissue cyst has grown, what you can very well see by the lack of margin around the Knochenzementplombe, the cement can be surgically removed and instead own bone from the iliac crest ( cancellous bone ) are introduced. After a long bony remodeling process ( remodeling ) the cyst region is fully loadable again.
