Sevoflurane (trade name Sevorane ) is a volatile anesthetic from the group of Flurane. It has a good hypnotic, but only weakly analgesic and muscle relaxant properties. The use of sevoflurane for anesthesia is widely used, especially in pediatric anesthesia.

Pharmacology

The blood - gas partition coefficient of sevoflurane is about 0.65, that is, at a concentration of 1 volume percent ( vol% ) in the pulmonary alveoli, the concentration is 0.65% by volume in the blood. The low solubility causes a rapid sleep and wake-up. The minimum alveolar concentration is 2% sevoflurane, making it less potent than isoflurane. Since sevoflurane is not irritating to mucous membranes and a pleasant " ethereal " has odor, it is also suitable for inhalational induction of anesthesia. This explains the frequent use in pediatric anesthesia.

The density of sevoflurane is as a liquid 1.52 g / ml, the boiling point is 58.5 ° C, the vapor pressure at 20 ° C of 23.1 kPa.

The metabolic capacity of sevoflurane is between 3 and 5%. This inorganic fluoride is released next hexafluoroisopropanol. Neither in healthy nor in renal patients an impairment of renal function ( nephrotoxicity ) by these metabolites could be shown. A special feature of sevoflurane is the reaction with the soda lime of the semi -closed anesthetic systems. Various degradation products arise ( Compound AE), are favoring a low fresh gas flow ( low-flow, minimal-flow anesthesia), dry soda lime, sodium hydroxide or barium containing the catalyst, as well as high gas concentrations. Compound A acts on animals in high concentrations nephrotoxic ( nephrotoxic ), but in clinical use in humans does not matter. Not have a harmful effect could be demonstrated for Compound BE. Sevoflurane is approved in Germany for low-flow and minimal-flow anesthesia without time limit.

Production

Syntheses of Sevoflurane are reported in the literature.
