Pharmaceutical care

The pharmaceutical care is the consistent perception of the co-responsibility of the pharmacist in drug therapy, with the aim to achieve tangible therapeutic results that are suitable to improve the quality of life of the patient.

The pharmaceutical care is a complementary offer of the pharmacist that supports the treatment of the physician to minimize drug- related problems.

Objectives

  • Optimization of drug therapy.
  • Identify and resolve drug-related problems ( ex. using the SOAP schema = Subjective and Objective gather information, analyze and plan = subjective objective assessment plan)
  • Assumption of responsibility by the pharmacist.
  • Assessment of the success subjectively from the point of view of the patient.
  • Cooperation with other health care professionals.
  • Creating a care plan
  • Improving the quality of life of the patient.
