Promazine is a heterocyclic aromatic compound is selected from the group of the thiazines. It is a weakly potent, classical neuroleptic from the group of phenothiazines. As a drug, it was marketed in Germany earlier under the name Protactyl and Sinophenin. It is one of the oldest drugs for the treatment of schizophrenia, but has been largely replaced by newer agents.

Indication

Psychotic states. Manic phase in bipolar disorder. Psychomotor agitation and tension, such as during an alcohol withdrawal. Specific forms of nausea and vomiting where drug of first choice are contraindicated or not have the expected effect.

Mechanism of Action

Promazine blocked like all phenothiazines a number of receptors for neurotransmitters, such as D2, α1 -, 5 -HT2, H1 and M1 receptors, causing an antipsychotic, sedative, anti -adrenergic, antihistaminic, anticholinergic, ganglion -blocking, anti-emetic and local anesthetic effect may occur. Promazine also acts as FIASMA ( functional inhibitors of acid sphingomyelinase ).

Adverse effects

As with all classical neuroleptics can extrapyramidal effects occur with promazine during treatment.

Frequently occur on daytime sleepiness, dry mouth, and blood pressure fluctuations as well as voiding dysfunction, constipation and tachycardia. These unwanted effects (side effects ) disappear when promazine in the course of treatment or they can be pushed back by a lowering of the dose.

Less frequently encounter interference of hematopoiesis, cholestasis, endocrine disorders, eczema and photosensitization, in individual cases, seizures, or glaucoma formation were observed. Also, dyskinesia, tardive dyskinesia after long-term treatment also may occur.

Very rarely, the development of neuroleptic malignant syndrome with fever, muscle stiffness, and altered mental status.

For long-term treatment and / or a high dose of promazine may lead to deposits in the cornea and lens of the eye.
