It rises in the Maya Mountains in the province of Toledo near the border with Guatemala and flows eastwards towards Caribbean Sea and ends at Monkey River Town in the Gulf of Honduras. The Monkey River has great significance for the downstream Belize Barrier Reef by many fish and other creatures benefit from sea water mixed with the clear water of the river.

Near the Monkey River are the Mayan ruins of Nim Li Punit Lubaantun and, at the lower reaches, the river flows through the Payne 's Creek National Park.
