On 13 February 2009, he was nominated by President Sharif Sheikh Ahmed as the new Prime Minister of the transitional government of Somalia. The transitional parliament elected him with 414 votes to 9 in this office.

The reasons for the choice Sharmarkes his clan membership are ( according to the Constitution President, Prime Minister and Speaker of Parliament must belong to three different major clans, and the Majerteen Darod were excreted in the resignation of President Abdullahi Yusuf Ahmed of the government), its links with the Somali Diaspora and to the United Nations, and the fact that he was never involved in the civil war in Somalia.

Sharmarke described the improvement of the humanitarian situation, the return of IDPs to their homes and reconciliation with the opponents of the Transitional Government as priorities.

Sharmarkes appointment was condemned by al -Shabaab. Sharmarke called for reconciliation and the creation of unity in Somalia. On September 21, 2010 Sharmarke resigned, citing a crisis in the government and the increasing insecurity in Somalia.
