Its headwaters is the Turquoise Lake in Lake Clark National Park. It flows southwest and empties in Koliganek, 105 km northeast of Dillingham, in the Nushagak River.

The used until the early 20th century by the Dena'ina Telaquana Trail runs between the Telaquana Lake and the Village at Lake Clark Kijik partially along the Mulchatna River. Today, the path is a trail for backpackers.

The first 39 km of the Mulchatna River within the national park in 1980 were reported by the Alaska National Interest Lands Conservation Act as a National Wild and Scenic River under the administration of the National Park Service.
