The first mention of a castellum Monticelli goes back to the year 1099. In the document of the monastery of Montecassino Crescenzo Duke of Fondi is referred as inhabitants of the castle. In 1145 it fell to the dell 'Aquila family, and as these became extinct in 1299 to their heirs the Caetani. More reigned over the town and castle, the Colonna ( 1494), Gonzaga ( 1570), Carafa ( 1591 ), Mansfield ( 1690) and di Sangro (1721 ). Despite the change of rulers remained under suzerainty of the Kingdom of Naples with importance as a border fortress of the Papal States above the Via Appia. In the district Epitaffio an inscription dating from the 16th century has been preserved on a watchtower. The customs post for the control of passengers and their cargo was located in the district but Portella where the watchtower and the gate are still visible under which the Via Appia ran. Last Prince of Monticelli was Giovanni di Sangro, as in 1860 the place was incorporated into the new Kingdom of Italy.

Because of this favorable position Monte San Biagio was plagued in its history again and again by brigands, especially in the years between 1861 to 1870 when Lazio and Rome were incorporated in Italy.
